When should a treatment center call us?

Core Values Recovery is the independent continuity team for families leaving treatment without an existing case manager. We coordinate the client, family, providers, accountability, and next level of care so discharge does not become a cliff.

Call CVR when

  • No independent case manager or interventionist owns aftercare.
  • Several providers and family members need one coordinator.
  • The family needs support without becoming the case manager.
  • Monitoring needs to connect to coaching and a response plan.
  • Repeated treatment or a complex transition makes a routine handoff insufficient.

Collaborate or redirect when

  • An engaged interventionist or case manager already owns continuity.
  • A structured program has its own case-management model.
  • The person needs emergency, detoxification, residential, or other acute clinical care.
  • The request is surveillance alone without consent and a recovery plan.

Our no-conflict commitment

We do not replace an engaged interventionist or case manager. When another professional remains involved, we clarify scope before beginning. Clinical decisions stay with clinicians. Communication follows the client's consent and an agreed level of detail.

The handoff

  1. Fit: Determine whether coordinated case management is appropriate.
  2. Define: Agree on roles, priorities, warning signs, and communication.
  3. Coordinate: Help the client and family execute the plan in daily life.
  4. Respond: Coordinate a change or return to care when the plan is no longer sufficient.
